ok guys, several of us have allergies/health issues that cause complications if you have an accident at a game. I know of at least two of us that have a so called ICE card, or In Case of Emergency.. this is a great idea for ALL of us, as any info you can give asap in an accident situation is worth its weight in gold or more. So, if you have templates or ideas for cards or even an ice kit lets have it, ps, thanks for the idea nyghtbringer

There are SO many ways to go here.. a few ideas are
the RED tags Blood type patches Allergy Ident pastches
IF you use these .. make certian that you know the inf is correct!!!! this is not the patch to wear because it's cool ... KNOW it's right!
As a SAR Team Medic I had all my crew's data in my left front BDU pocket ... and wore a contact tag on my gear harness and one in my left boot laces.. and used it later when we were mobilized.
If you have injectables PLEASE tell two people what it is , were it is and how to use it if needed .
I think I still have my RED Record of Emergency Data sheets from a former life here at Camp Warrior someplace .. I'll look and see

i have a small card that i keep in my "medic" pouch. i usually let someone on my "team/squad" know where its at in case of emergency.
EMERGENCY CONTACT
ON-SITE: name (handle) (relation) (xxx) xxx-xxxx
OFF-SITE: #1. name (relation) CELL (xxx) xxx-xxxx
#2. name (relation) CELL (xxx) xxx-xxxx HOME (xxx) xxx-xxx
BLOOD TYPE: xxxxxxxxx ALLERGIES: xxxxxxx (abrieviation if known)

hmm i have an idea i thought of this am, and am exploring it further, where i work had an embossing machine at one time used to make ad hoc id cards for clients, its is not used any more and i am in search of our physical assets guy to see if i can use it for this purpose.. ill keep you posted, but we need to come up with the best arrangement of info on the card as possible for space and convienience considerations..

while the phone is not a bad idea. some phones have passcodes to unlock them. some people do not carry thier phones onto the field. the bloodtype attachments cost money.
keeping a small card with the above mentioned info can fit almost anywhere. it only requires a computer and printer. if you want to laminate it you can (i suggest you do). but you don't have to.
i'll be making a second card for when my buddy doesnt make a game.
basic info needed. 1.your full name 2. off-site emergency contact (preferably 2) include name, relation, phone number (cell/home/work) 3. blood type 4. allergies

Who all on here are qualified to be field medics? I'm not asking if you have a phd, just fellas who will know how to handle minor emergencies such as a sprung ankle. That way we will know who can be medics on the field.
I myself am certified. Who else?

I was certified 19 years ago! Lol. I'm pretty sure my certification has run out! BUT.....with drag racing experience and well over 2,000,000 certified career miles, I'd make one heck of an ambulance driver!!
